Create a service template

Give your team consistent prompts for progress notes, incident reports and other case-note writing.

Where to find it

Services → select a service → Templates

Before you start

You need permission to manage templates for the service. Templates belong to a service, so start with the service whose team will use this writing guide.

Steps

  1. Create the template

Open the service’s Templates tab and choose Create template. Give it a recognisable Template name, such as Progress note or Incident report.

  2. Add useful sections

Choose Add section for each part of the note. Enter a Section label and optional Help text explaining what to write. For an incident report, your team might use sections for what happened, immediate actions and follow-up.

  3. Choose required responses

Use Required before submission for sections that must be completed. Keep prompts brief and specific so staff know what information is expected, without repeating the same request in several sections.

  4. Preview and set the default

Arrange the sections in the order staff will use them and check the Preview. If this should be the starting template for new notes in the service, select Default for new notes in the template details.

  5. Save and share the location

Choose Save template. Staff can select it under Case note template after choosing this service in a new case note. Let your team know its name and when to use it.
