Use a case note template
Choose your service’s writing guide when creating a progress note, incident report or another case note.
Where to find it
Notes & charges (or Case notes) → Create case note
Before you start
You need permission to create a case note for the participant and service. A service manager must make the relevant template available; the app does not automatically supply your organisation’s incident-report format.
Steps
Select the participant and service
Choose Create case note and complete the Participant and Service selections. The Case note section and the sections below it appear once both are selected.
Choose a template
In Case note template, select the template you need. The service’s default may already be selected. Search the available choices, or choose Standard form if you do not need a custom template.
Write under each prompt
Complete the template sections using their headings and help text. For an incident report, follow your organisation’s recording and reporting procedures as well as completing the note.
Finish the record
Check session details, required responses and any attachments or charges that apply. Save a draft if you need to finish later, or submit when the record is ready for review.
Ask for a missing template
If the template you need is absent, check that you chose the correct service. Ask someone with template-management permission to create it in that service’s Templates tab.
